Counted straight from the database: every fix applied, every time Daryl rolled one back because error rates rose, and every time a customer pressed undo. Nothing is sampled or estimated.

Definitions
What counts
- Applied
- A fix that changed a customer’s app, by one-click apply, a merged pull request or Autopilot.
- Rolled back
- Daryl restored the exact prior state on its own because error rates rose during the watch window.
- Undone
- A customer pressed undo within 30 days. Counted as breakage even when the reason was something else.
- Breakage
- Rolled back plus undone, divided by applied.
